以Hewlett Packard,Model Sonos 1000之5.5 MHz探頭,探測四例眼窩腫瘤,兩例眼窩囊腫,及一例眼內腫瘤。灌輸至腫瘍之動脈及中心視網膜動脈可於彩色都卜勒血流圖上指認。並可導出搏動指數及阻力指數。於彩色都卜勒血流圖,眼窩電腦斷層掃瞄,抑或核磁共振顯像圖之導引下,七位患者之腫瘍得以手術摘除,檢體並送病理診斷。
兩例眼肌錐束內之腫瘤為原發性之竇狀血管瘤。兩例眼肌錐束外之腫瘤分別為自胃部轉移之分化不良腺癌,另一例為源自淚腺之類淋巴增生(亦稱偽瘤)。一例囊腫為源自淚囊之黏液囊腫,另一例則為非角化眼窩囊腫。眼內腫瘤則為黑色素瘤。血流動力學研究顯示;黑色素瘤及偽瘤之灌輸動脈為高流速,竇狀血管瘤及非角化眼窩囊腫則為中流速,轉移腫瘤及黏液囊腫為低流速。
就眼科學及病理學而言,本研究乃首先嘗試對於眼肌錐束內外及眼球內之腫瘍,量化其血流動力學。彩色都卜勒血流圖之特異性及敏感性是有意義的,但仍須再進一步之檢驗。
Ophthalmologically, CDI not only provides the morphogical diagnosis of a mass lesion and its blood supply, but also quantitates the hemodynamics.
CDI (Hewlett Packard, Model sonos 1000) with a 5.5 MHz probe was applied to four orbital tumors, two orbital cystic lesions, and one intraocular tumor. The feeding arteries (FAs) and central retinal arteries (CRAs) were identified on CDI. Pulsatility and resistivity values were then calculated. Under guidance of CDIs, orbital computerized tomographs, and/or magnetic resonance imaging (MRI), all patients underwent surgical extirpation and specimens were submitted for pathological studies.
Two intraconal tumors were shown to be primary cavernous hemangiomas. One extraconal tumor was shown to be a metastatic tumor from poorly differentiated gastric adenocarcinoma. The other extraconal mass was shown to be lymphoid hyperplasia (pseudotumor) from the lacrimal gland. One cystic lesion was shown to be a dermoid cyst. The intraocular tumor was shown to be a melanoma. Hemodynamic studies demonstrated that the feeding arteries of the melanoma and pseudotumor were high flow, while those of the cavernous hemangioma and nonkeratinizing orbital were medium flow, and those of the metastatic tumor and mucocele had the lowest blood flow velocities.
Ophthalmologically and pathologically, this study is the first attempt to try to quantitate the hemodynamics of intraconal, extraconal and intraocular mass lesions. The specificity and sensitivity of CDI were significant but still need refining.
